Because coverage can vary by plan,
we recommend contacting your insurance provider to confirm:​

PATIENT RESPONSIBILITY / COST
Patients are responsible for all copays, deductibles, and coinsurance required by their insurance plan.

At the time of your visit, please be prepared to:

Any remaining balance after your insurance processes your claim will be billed to you.

After your visit:

We submit your claim to your insurance company

Your insurance reviews and processes the claim

You will receive an explanation of benefits (EOB) from your insurer

If a balance remains, you will receive a statement from our office
If you have questions about your statement or EOB, our billing team is available to assist.
